Our Dementia Support Advisors are experienced in supporting people living with dementia and their carers at every stage in the condition. From someone who is concerned about confusion or memory loss, or who has been newly diagnosed, to those who need residential care, the team will be there, offering support, advice, signposting, practical suggestions or simply a listening ear.
Home Visit
Dementia Support Advisors offer an initial home visit to meet and discuss the individual needs of anyone who is referred, or self refers, to Dementia Forward. During the visit they will talk about the range of services offered by us and other organisations and find out what may be of interest. They will spend time with the person with dementia and their family, giving everyone the opportunity to find out information they want to know, when they want to know it.
Dementia Forward Support Advisors are flexible to the needs of the individual, and the support you receive will be tailored to you, but they may help you to:
• Understand dementia and what might happen in the future
• Choose the types of services and support that best meet your needs
• Stay connected to your local community by supporting you to take part in the range of wellbeing services
• Make informed decisions about your care, treatment, wellbeing and support needs
• Discover any financial benefits you may be entitled to
Dementia Forward will not discharge anyone from their support, unless they request it of course, and after the initial visit the Support Advisors will be contactable by phone and at wellbeing services.
